Requirements
- CPSO Certificate of Registration for Independent Practice (unrestricted), or restricted under CPSO's US-trained pathway
- CCFP or ABFM board certification in family medicine
- Active OHIP billing number
- CMPA coverage in good standing
- 2+ years as a family physician, currently practising in Ontario
