You start with a full roster.
At least 1,200 verified, waitlisted patients available to each doctor, ready to attach on day one. You decide how large your roster gets. You will see the patient waitlist during the selection process.
Button Health is a family practice at the corner of Bay and College in downtown Toronto. Continuity care only. No walk-in. No on-call.

How the practice runs
- Trained MOAs, an assistant, full-time off-site call answering, and an experienced office manager
- You set your schedule. Three, four or five clinical days a week.
- Online booking and prepared intake. Patients arrive ready. Substantial messaging to patients about how Canadian healthcare works and expectations set on OHIP care.
- All tools, supplies and equipment provided, including the EMR. Third-party OHIP billing available if you need it.
What you are not doing
- Not waiting years to build a roster. We spend heavily on patient acquisition, and it's ongoing, not a one-time event.
- Not training staff, not teaching staff, not dealing with front office issues.
- Not waiting for the EMR and internet to work. Double redundant, enterprise-level network set up by a professional IT management company.
- Not being capped at OHIP.
Compensation
The clinic will operate under a fee-for-service (FFS) Family Health Group (FHG) model. We are seeking a future transition to FHO+.

- $420,000 plus a year (OHIP & block fees)

- Target $84,000 a year, scaled to days worked (on top of your OHIP/block fee earnings).
Practice support
- Clinic operations, staffing, and administrative systems handled by Button Health
- Two full-time in-office MOAs, virtual administrative support, and a clinical assistant
- Online booking, patient self-check-in, and a patient portal for results and automated follow-up
- A verified patient waitlist, with roster growth paced by the physician and supported by the clinic
- Patient acquisition, roster management, growth, and marketing managed and paid for by the clinic
- Planned time away coordinated by the clinic and covered within the physician group once the team is in place
- Space, EMR, supplies, and equipment provided
- Clinic-arranged third-party billing support is available if wanted
Professional support
Annual professional allowance
Up to $5,000 annually for CME, conferences, CPSO, CFPC and OMA dues, and licensing.

Gym pass
A gym pass is included for physicians practising at Button Health.
Physician referrals
Physician referral award
Receive a $20,000 award for a successful physician referral.
The space
An existing 1,700 sq ft medical office being refreshed before opening, with seven exam rooms, a doctors' office and a full lunch room. Lab, imaging, cardiology and sports medicine in the same building. Subway station a block away.
Private Practice Path
Button Health physicians have an earned, structured pathway into private general practice (concierge medicine) through our affiliated clinics. It is optional. If OHIP family practice is the whole of your plan, this practice stands on its own.
Responsibilities
- Continuous, comprehensive primary care to the patients on your roster, including diagnosis, treatment, and ongoing management of acute and chronic conditions.
- Physical examinations, routine health assessments, and preventive care.
- Ordering, interpreting, and following up on laboratory tests and diagnostic imaging.
- Prescribing and administering medications and treatments in line with current best practice.
- Primary care for the clinic's patient population, including ADHD and PrEP care where it falls within your scope and clinical judgment.
- Complete, accurate clinical records in the practice EMR.
Requirements
- Family medicine certification: CCFP (Canada, any province), MRCGP with CCT (UK), ABFM (USA), MICGP (Ireland) or FRACGP (Australia)
- CPSO registration permitting independent practice, or eligibility for it
- Eligible for CMPA coverage
International physicians
UK, US, Irish and Australian family physicians with recognized training and certification may qualify for CFPC certification without examination. With that certification, eligible physicians can obtain CPSO provisional registration to practise independently within their scope without Canadian exams. We handle the LMIA and work permit.
Not eligible: Physicians who still need to complete Canadian residency training or supervised practice, PA candidates, medical students.
